Plaque is constantly forming, and if feeds off of sugars or other fermentable carbohydrates in your diet. It develops a film on your teeth within 24 hours of cleaning them. If not removed within 72 hours, it mineralizes with phosphate, calcium, and other elements. Mineralized plaque, known as tartar or calculus, is hard and therefore very difficult to remove at home with a toothbrush or floss.

You can minimize plaque buildup by brushing your teeth at least twice a day and for two minutes. Be sure to brush properly and get the brush head all the way to the gumline. This is the most often neglected area, but is considered the most important. Only use a soft brush and avoid overly abrasive toothpastes, as they could damage enamel. If you brush thoroughly, you will only be able to clean about two thirds of the tooth surface. This is where Floss comes in. Flossing daily is vital to clean the sides of teeth and other areas that a brush will not reach.

Unfortunately, it is highly unlikely that you will be able to remove 100 percent of the plaque with brushing and flossing alone. Any exposed surfaces below the gumline are difficult to access, as are tight spaces between the teeth. Additionally, teeth may have deep fissures that are large enough to harbor bacteria, but too small to clean with a brush. Gingivitis (the earliest form of gum disease) can be cured, often with no treatment beyond a thorough cleaning and enhanced oral hygiene techniques. Similarly, incipient caries (early decay, confined to the enamel layer) may remineralize with thorough cleaning and treatments such as fluoride.
